Our goal in this blog is to promote a Triple Approach to the Earth Citizens' Health. This Triple Approach consists of : 1) Quaternary Prevention (Preventing Unnecessary Interventions in Clinical Medicine and Public Health ), 2) Prevention of Diseases and Treatment of Patients by Improving Living Conditions and Social Prescribing, 3) Preconception Prevention of Diseases by Preconception Planning.

Study: Pharmacists completing medication management plans in the discharge summary significantly reduced the rate of medication errors (including errors of high and extreme risk) in medication summaries for general medical patients.
